Daybreak are a champion of person centred approaches and have embedded new practices throught the organisation. Daybreak have introduced one page profiles, person centred action plans and reviews as well as a number of creative person centred thinking tools. These new practices have been introduced to everyone from the people we support to staff, trustees and volunteers, ensuring that all Daybreak beneficiaries receive a service which meet their individual developing and changing needs.
By working in this way we can make sure people are truly listened to and are kept at the heart of all decision making and how sevices are provided and organised.
Person centred practices are used in teams to ensure that we focus on what matters to the people we support, their families and pay attention to how we support our staff as well.
