Person Centred Plan Montagu
Daybreak supporting Adults with Learning Disabilities - Person Centred Plan
On Tuesday 19th April, Reporters from the Montagu Hub interviewed Robert T about his recent Person Centred Plan. This is what Robert told them:
'I like coming to Daybreak because I have made new friends and I like going to different places. I like to see the staff and all my friends when I am at Daybreak. I enjoy the gym, swimming and going out somewhere with Adam and Lorna (LSC's at Montagu Hub).
On Wednesday (20th April) I am performing at the Gala Theatre with my friends. The performance is all about coal mining and I am playing a part of a miner. I am looking forward to it but I am a little bit nervous performing on stage with my friends.
I felt my Person Centred Plan was brilliant, Adam (LSC) asked many different questions and it was a way for me to tell Adam important things about my family, friends, carers and what I would like to work towards.
One of my goals is road safety. I would like to be able to recognise when it is safe to cross the road safely and I am hoping with the help and support from the staff at Daybreak I will be able to achieve my goal.'
